Form I-9, Section 1 Federal · required
Shows you are allowed to work in the U.S. Fill out and sign Section 1 only, no later than your first day. On your first day, bring original, unexpired ID documents from the I-9 list (for example a U.S. passport, or a driver's license plus a Social Security card). We complete Section 2 with you in person.
Form W-4 Federal tax · required
Tells us how much federal income tax to withhold from your pay. The Spanish version is for reading along; fill out and sign either one.
Form IT-2104 New York tax · required
Tells us how much New York State income tax to withhold. Available in English only. Ask us if you need help with it.
Pay notice (LS 54) New York · we send it
New York law requires us to give you a written notice of your pay rate and payday, in English and in your main language. We fill it out and send it to you. Sign it and upload it here, or hand it back to us.

Sign the acknowledgment pages we give you, including our sexual harassment prevention policy. Direct deposit is optional. Upload the signed pages under "Other signed forms".
